Sec. 5.7. The permittee must notify the commissioner a minimum of ten (10) days, excluding Saturdays, Sundays, and state of Indiana holidays, before exercising a permit issued by the commissioner in accordance with this rule. The notification must include the following information:

1. (1) The construction permit number assigned by the commissioner.
2. (2) The location of the construction.
3. (3) A description of the construction.
4. (4) Anticipated duration of the construction.
5. (5) The phone number of the permittee or permittee's representative who will be present during the construction.
