Sec. 17. (a) A wastewater treatment plant operator certificate:

1. (1) is issued after an applicant successfully completes the examination for the appropriate classification;

2. (2) must specify the:

   1. (A) month and year that the applicant qualified; and
   2. (B) date the certificate was issued;
3. (3) must be validated by a current certification card; and

4. (4) is not valid if obtained through:

   1. (A) fraud;
   2. (B) deceit; or
   3. (C) the submission of incomplete or inaccurate data on the examination application.

(b) A certification card:

1. (1) may be issued for a time period of not more than thirty-six (36) months; and
2. (2) expires on the date printed on the card.

(c) To obtain a replacement or duplicate certificate, a certified operator shall submit a written request to the commissioner, including the following information:

1. (1) The class of the wastewater treatment plant operator.
2. (2) The date of issuance of the original certificate, if known.
3. (3) The certificate number.
