Sec. 4.2. The commissioner or the commissioner's designated agent may revoke an approval letter if the applicant:

1. (1) violates any requirement of section 4.1(d) of this rule;
2. (2) violates any condition added to the approval letter under section 4.1(f) of this rule; or
3. (3) falsifies information on an application for an approval.
