Sec. 7. (a) To renew CLEAN membership, a member of senior management shall submit the fourth annual summary of the previous membership term:

(c) The department shall review the annual summaries and four (4) objectives, targets, and action plans submitted during the most recent term of membership. The review of this information shall follow the same procedures as the review for the initial application regarding compliance checks.

(e) If the entity does not request a renewal with the fourth annual summary, then membership in CLEAN expires upon receipt by the department of the fourth annual summary.
