Sec. 3. (a) In addition to the information required under section 2 of this rule, the department may require the following information to be included in an application:

1. (1) The name and address of any adjacent property owner, municipality, or other interested person who may be affected by placement of the navigation aid or water recreation structure.
2. (2) Information that will assist in determining whether issuance of the permit is likely to result in any hazard to persons, property, or the environment.
3. (3) Any other information deemed appropriate by the director.

(b) The department shall require an applicant to provide notice to any person that will be affected by issuance of a license.

(c) A consolidated application may be made for a license to govern all navigation aids or water recreation structures (or all navigation aids or water recreation structures of a particular type) on a specified public freshwater lake, navigable waterway, or segment of a public freshwater lake or navigable waterway.
