Sec. 8. (a) A license holder must report the results of the event on a form provided by the department within thirty (30) days of the completion of the event, including the following:

1. (1) The number of boats and participants.
2. (2) The starting and ending times.
3. (3) The actual use of public facilities.

(b) In addition to the terms of the license and the requirements otherwise set forth in this rule, the department may require a fishing tournament license holder to keep and report by a specified date, on a department form, legible and accurate records of the following:

1. (1) The tournament name.
2. (2) The name, address, and telephone number of the license holder.
3. (3) The tournament date or dates, including starting time and ending time.
4. (4) The target fish species.
5. (5) The name of any waterway fished.
6. (6) The number of boats and number of participants.

7. (7) Individual or team catch statistics for each species of fish taken, including the following:

   1. (A) The numbers and lengths of fish weighed-in.
   2. (B) The numbers and lengths of fish caught and released.
