Sec. 6. A person, other than the person whose permit was revoked, may obtain a permit for a well revoked under IC 14-37 and section 5 of this rule if the person assumes full responsibility for:

1. (1) abatement of the violation; and
2. (2) operation, maintenance, and plugging of the well.
