Sec. 8. An employee who meets all the following criteria is entitled to long-term disability benefits:

1. (1) The employee must be employed on a full-time basis and have six (6) months of continuous full-time employment prior to the beginning of the elimination period.
2. (2) The employee must be absent due to illness, injury, or legal quarantine as evidenced by a physician's signed statement.
3. (3) The employee must have been absent for a continuous period of six (6) months due to the disability.
