Sec. 7. (a) Written policy, procedure, and practice provide that social services staff are to be available to meet with juveniles:

1. (1) on a regularly scheduled basis at least once a month; and
2. (2) at the juvenile's request.

(b) Individual contacts are documented in the form of a contact note in the juvenile's record.
