Sec. 1. In addition to all other legal requirements, an individual applying for an Indiana driver's license after the individual's Indiana license expired must do the following:

1. (1) If the license expired not more than one hundred eighty (180) days before the current application, the individual must pass an eyesight examination or provide a vision screening completed by an ophthalmologist or optometrist under IC 9-24-10-4(c)(1).

2. (2) If the license expired more than one hundred eighty (180) days but not more than three (3) years before the current application, the individual must pass:

   1. (A) an eyesight examination or provide a vision screening completed by an ophthalmologist or optometrist under IC 9-24-10-4(c)(1); and
   2. (B) the written examination or examinations applicable to the type of license and endorsement sought.

3. (3) If the license expired more than five (5) years before the current application, the individual must pass:

   1. (A) an eyesight examination or provide a vision screening completed by an ophthalmologist or optometrist under IC 9-24-10-4(c)(1);
   2. (B) the written examination or examinations applicable to the type of license and endorsement sought; and
   3. (C) a driving skills examination.
