Sec. 19. A vehicle's certificate of registration shall contain at least the following information:

1. (1) The name and address of the person leasing the vehicle or who owns the vehicle.
2. (2) The license plate number assigned to the vehicle.
3. (3) The make and model of the vehicle.
4. (4) The dates for which the registration of the vehicle is valid.
