In addition to the notification required by section 1 of this chapter, the coroner or other official performing similar functions shall report in writing to the state police department, on or before the tenth day of each month, the following:

1. (1) The death of an individual within the coroner's jurisdiction during the preceding month as the result of an accident involving a motor vehicle.

2. (2) The circumstances of the accident.
