1. (a) Any male who:

   1. (1) applies for issuance or renewal of a driver's license;
   2. (2) is less than twenty-six (26) years of age; and

   3. (3) is or will be required to register under 50 U.S.C. App. 453(a);

      may authorize the bureau to register him with the Selective Service System in compliance with the requirements of the federal Military Selective Service Act under 50 U.S.C. App. 451 et seq.

2. (b) The application form for a driver's license or driver's license renewal must include a box that an applicant can check to:

   1. (1) identify the applicant as a male who is less than twenty-six
   2. (26) years of age; and
   3. (2) indicate the applicant's intention to authorize the bureau to submit the necessary information to the Selective Service System to register the applicant with the Selective Service System in compliance with federal law.

3. (c) The application form for a driver's license or driver's license renewal shall contain the following statement beneath the box described in subsection (b):

   "Failure to register with the Selective Service System in compliance with the requirements of the federal Military Selective Service Act, 50 U.S.C. App. 451 et seq., is a felony and is punishable by up to five (5) years imprisonment and a two hundred fifty thousand dollar ($250,000) fine. Failure to register may also render you ineligible for certain federal benefits, including student financial aid, job training, and United States citizenship for male immigrants. By checking the above box, I am consenting to registration with the Selective Service System. If I am less than eighteen (18) years of age, I understand that I am consenting to registration with the Selective Service System when I become eighteen (18) years of age.".

4. (d) When authorized by the applicant in conformity with this section, the bureau shall forward the necessary registration information provided by the applicant to the Selective Service System in the electronic format or other format approved by the Selective Service System.
5. (e) Failure of an applicant to authorize the bureau to register the applicant with the Selective Service System is not a basis for denying the applicant driving privileges.
