1. (a) The following may apply for a permit under this chapter on a form and in the manner prescribed by the appropriate permit authority:

   1. (1) A person that provides wireless communications service.
   2. (2) A person that owns or otherwise makes available infrastructure required for wireless communications service.

2. (b) An application for a permit must include the following:

   1. (1) The name, business address, and point of contact for the applicant.
   2. (2) The location of the proposed or affected wireless support structure or wireless facility.
