As used in this chapter, "rates and charges", with respect to basic telecommunications service, means the monthly charge to a customer for basic telecommunications service, including:

1. (1) recurring charges for flat rate and message rate service; and
2. (2) any nonrecurring charge for installation or a line or service connection.
