As used in this chapter, "qualified higher deductible health plan" means a health coverage policy, certificate, or contract that:

1. (1) provides for the payment of eligible medical expenses that exceed a higher deductible;
2. (2) is purchased by an employer for the benefit of employees and dependents of employees; and
3. (3) is part of a medical care savings account program established under this chapter.
