Project IMPACT USA, Inc., is authorized to develop and implement the following five (5) affiliate organizations to be administered at the following sites:

1. (1) Project IMPACT Allen in Fort Wayne.
2. (2) Project IMPACT Lake in Gary.
3. (3) Project IMPACT Marion in Indianapolis.
4. (4) Project IMPACT St. Joseph in South Bend.
5. (5) Project IMPACT Vanderburgh in Evansville.
