Ind. Code § 4-33-22-15

Applications

As added by P.L.113-2010, SEC.11.
  1. (a) Applications for licenses or permits to conduct or participate in, either directly or indirectly, a boxing, sparring, or unarmed combat match or exhibition must be:

    1. (1) made in writing upon forms prescribed by the commission and shall be addressed to and filed with the gaming commission; and
    2. (2) verified by the applicant, if an individual, or by an officer of the club, corporation, or association in whose behalf the application is made.
  2. (b) The application for a permit to conduct a particular boxing, sparring, or unarmed combat match or exhibition must, among other things, state:

    1. (1) the time and exact place at which the boxing, sparring, or unarmed combat match or exhibition is proposed to be held;
    2. (2) the names of the contestants who will participate and their seconds;
    3. (3) the seating capacity of the buildings or the hall in which such exhibition is proposed to be held;
    4. (4) the proposed admission charge;
    5. (5) the amount of the compensation percentage of gate receipts that is proposed to be paid to each of the participants;
    6. (6) the name and address of the applicant;
    7. (7) the names and addresses of all the officers if the applicant is a club, a corporation, or an association; and
    8. (8) the record of each contestant from a source approved by the commission.
  3. (c) The commission shall keep records of the names and addresses of all persons receiving permits and licenses.
