Ind. Code § 33-42-4-2

Fraudulent acts

Sec. 2. A person authorized to administer oaths or take acknowledgments who, with intent to defraud:

  1. (1) affixes the person's signature to a blank form of affidavit or certificate of acknowledgment; and
  2. (2) delivers that form to another person, with intent that it be used as an affidavit or acknowledgment;

    commits a Level 6 felony.
