A notice of lien recorded under this chapter must:

1. (1) state:

   1. (A) the name of the claimant;
   2. (B) the name of the owner of the commercial real estate upon which the lien is claimed;
   3. (C) a legal description of the commercial real estate upon which the lien is claimed;
   4. (D) the amount for which the lien is claimed; and
   5. (E) the license number of the broker company's license under IC 25-34.1 ;
2. (2) contain a statement that the information contained in the notice is true and accurate to the knowledge of the signatory;
3. (3) be signed by the managing broker responsible for the actions of the broker company or by a person authorized to sign on behalf of the managing broker; and
4. (4) be verified.
