A petition seeking to restrain a person from contact must be entitled "In the Matter of a No Contact Order for ___________". The petition must allege the following:

1. (1) That the respondent is likely to have direct or indirect contact with the child in the absence of an order under this chapter.
2. (2) That the child has been adjudicated a delinquent child.
3. (3) That the best interests of the child will be served if the person refrains from direct or indirect contact with the child.
