Not later than ninety (90) days after the date on which the department's report for a case involving a child fatality or near fatality is completed, the department shall provide to the public a summary report that includes the following:

1. (1) Any actions taken by the department in response to the case, including changes in policies or practices that have been made to address any issues raised in the review of the case.
2. (2) Information prescribed in section 4 of this chapter.
