"Unknown or insufficient address" refers to an address on a mailing returned by the United States Postal Service with one (1) or more of substantially the following statements made on the mailing by the United States Postal Service or a person residing at the address to which the mailing was sent:
- (1) Attempted-Not Known.
- (2) Box Closed.
- (3) Deceased.
- (4) Forwarding time expired.
- (5) Insufficient Address.
- (6) Left No Address.
- (7) Moved.
- (8) No such number.
- (9) No such office in state.
- (10) No such street.
- (11) Not deliverable as addressed.
- (12) Refused.
- (13) Returned for better address.
- (14) Returned to sender.
- (15) Unable to forward.
- (16) Unclaimed.
- (17) Undeliverable as addressed.
- (18) Vacant.
