"Ballot" means:

1. (1) the paper ballot prepared, printed, and supplied for use at an election;
2. (2) the electronic display prepared, printed, and supplied for use on the front of an electronic voting system; or

3. (3) the ballot card prepared, printed, and supplied for use in a ballot card voting system.
