As used in this chapter, "insured" means an individual who is entitled to the benefits provided by a health insurance policy. The term includes the following:

1. (1) A policyholder of an individual health insurance policy.
2. (2) A member of the group covered by a group health insurance policy.
3. (3) A female who is entitled to coverage under a health insurance policy as a spouse or dependent of an individual referred to in subdivision (1) or (2).
