1. (a) As used in this chapter, "provider" means:

   1. (1) a dentist licensed under IC 25-14 ; or
   2. (2) a dental office through which one (1) or more dentists licensed under IC 25-14 provide dental services.
2. (b) The term does not include a physician organization or physician hospital organization that leases or rents the network of the physician organization or physician hospital organization network to a third party.
