As used in this chapter, "dental carrier" means any of the following:

1. (1) An insurer that issues a policy of accident and sickness insurance that covers dental services.
2. (2) A health maintenance organization that provides, or provides coverage for, dental services.

3. (3) An entity that:

   1. (A) provides dental services; or

   2. (B) arranges for dental services to be provided;

      but is not itself a provider.
