Ind. Code § 27-7-17.5-6
"Provider"
As added by P.L.42-2022, SEC.1.
As used in this chapter, "provider" means:
(1) a dentist licensed under IC 25-14 ; or
(2) a dental office through which one (1) or more dentists licensed under IC 25-14 provide dental services.
