As used in this chapter, "covered service" means a dental service for which a reimbursement:

1. (1) is available under a dental plan; or

2. (2) would be available under a dental plan but for the application of contractual limitations such as:

   1. (A) deductibles;
   2. (B) copayments;
   3. (C) coinsurance;
   4. (D) waiting periods;
   5. (E) annual or lifetime maximums;
   6. (F) frequency limitations;
   7. (G) alternative benefit payments; or

   8. (H) any other limitation;

      under the dental plan.
