As used in this chapter, "provider billing agent" means a person or entity that contracts with a dental provider to provide billing services for the dental provider, including:

1. (1) submitting bills to patients;
2. (2) requesting payment from patients; and

3. (3) receiving payment from patients;

   under the terms and conditions of the contract between the dental provider and the person or entity.
