Ind. Code § 27-4-10-3
"Dental provider"

As used in this chapter, "dental provider" means:
- (1) a dentist licensed under IC 25-14 ; or
- (2) a dental office that provides billing and administrative services for one (1) or more dentists licensed under IC 25-14 who provide dental care through the dental office.
