Subject to sections 10 and 11 of this chapter:

1. (1) a group or an individual contract;
2. (2) an evidence of coverage; or

3. (3) an amendment to:

   1. (A) a group or an individual contract; or

   2. (B) an evidence of coverage;

      may not be delivered or issued for delivery in Indiana unless the form has been filed with and approved by the commissioner.
