A health maintenance organization shall provide a completed health maintenance organization comparison sheet to the following:

1. (1) Upon request, to an enrollee or subscriber or to the enrollee's or subscriber's employer.
2. (2) As part of the health maintenance organization's marketing materials, to a person or an employer that may be interested in purchasing or obtaining coverage under a group contract or individual contract offered by the health maintenance organization.
