In addition to the fees required by section 1 of this chapter, a health maintenance organization subject to this article must pay the fees required by IC 27-1-3-15 . For purposes of IC 27-1-3-15 , a contract between a health maintenance organization and a subscriber or enrollee and an evidence of coverage shall constitute a single policy.
