"Carrier" refers to any of the following:
- (1) A health maintenance organization.
- (2) An insurer licensed in Indiana to write Class 1(B) or Class 2(A) lines of insurance.
- (3) Any other entity responsible for the payment of benefits or provision of services under a group contract.
