1. (a) This section does not apply to the following:

   1. (1) A state employee health plan (as defined in IC 5-10-8-6.7 (a)).
   2. (2) The Medicaid program.

2. (b) A utilization review entity may not require prior authorization for the first twelve (12):

   1. (1) physical therapy; or

   2. (2) chiropractic;

      visits of each new episode of care.
