As used in this chapter, "foreign insurer" means the following:

1. (1) An insurer that is:

   1. (A) licensed to do business in Indiana under IC 27-1-17 ; but
   2. (B) not a domestic insurer.

2. (2) A health maintenance organization that:

   1. (A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and
   2. (B) has obtained a certificate of authority under IC 27-13-2 .

3. (3) A limited service health maintenance organization that:

   1. (A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and
   2. (B) has obtained a certificate of authority under IC 27-13-34 .

4. (4) A fraternal benefit society that:

   1. (A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and
   2. (B) has obtained a certificate of authority under IC 27-11-8-5 .
