Ind. Code § 27-1-1.5-10 – "Annual Statement Blank"

"Annual Statement Blank" refers to the document entitled "Annual Statement Blank" that is:

(1) adopted by the NAIC;
(2) in effect on January 1, 2018; and
(3) applicable to the kind of insurance to which the information contained in the document applies.
