A licensee shall:

1. (1) conspicuously display the licensee's license in the licensee's main office and at each facility included under the license;
2. (2) conspicuously display in each operational office the approved schedule of charges for services; and

3. (3) conspicuously display at each facility all charts and diagrams provided to the facility by the agency.
