The notice of proposed rulemaking must include all of the following:

1. (1) The proposed time, date, and location of the meeting at which the rule will be considered and voted on.
2. (2) The text of the proposed rule or amendment and the reason for the proposed rule.
3. (3) A request for comments on the proposed rule from any interested person.

4. (4) The manner in which an interested person may submit:

   1. (A) notice to the commission of the interested person's intention to attend the public hearing; and
   2. (B) written comments.
