A loan broker agreement that is delivered or required to be delivered by a person licensed or required to be licensed under this chapter to a borrower or prospective borrower must contain:

1. (1) the license number of the loan broker; and

2. (2) the license number of each:

   1. (A) mortgage loan originator; or

   2. (B) principal manager;

      who had contact with the file.
