Sec. 8. The secretary of state shall prepare certificates of merger that specify the following:

1. (1) The name of each party to the articles of merger.
2. (2) The name of the successor and the location of the successor's registered office in Indiana.
3. (3) The date the articles of merger are accepted for record by the secretary of state.
