1. (1) The foreign corporation's corporate name.
2. (2) The period of the foreign corporation's duration.
3. (3) The state or country of the foreign corporation's incorporation.
4. (b) The requirements of section 3 of this chapter for obtaining an original certificate of authority apply to obtaining an amended certificate under this section.

Sec. 4. (a) A foreign corporation authorized to transact business in Indiana must obtain an amended certificate of authority from the secretary of state if the corporation changes any of the following:
