- (a) A person that operates a CMRA in Indiana that knows or reasonably should know that an address that the CMRA has provided has been used as a contact address shall notify the secretary of state of the closure of a CMRA account not later than sixty
- (60) days after closure of the CMRA account.
(b) The notification described in subsection (a) shall include the:
- (1) physical address provided by the CMRA customer on the United States Postal Service Form 1583; and
- (2) electronic mail address associated with the CMRA account.
