The department shall require each postsecondary proprietary educational institution to include in each curriculum catalog and promotional brochure the following:

1. (1) A statement indicating that the postsecondary proprietary educational institution is regulated by the department under this chapter.
2. (2) The department's mailing address and telephone number.
