Each private postsecondary educational institution and each out-of-state public and nonprofit degree granting institution that offers instructional or educational services or training in Indiana shall provide to the department, in a manner, form, and frequency prescribed by the department, the following:

1. (1) Information necessary, as determined by the department, to carry out IC 20-19-3-22.3 .
2. (2) Any information the department requests to carry out IC 20-31-8-5.5 .
