The centers shall be known as the following:

1. (1) Indiana University School of Medicine-Northwest-Gary.
2. (2) Indiana University School of Medicine-Fort Wayne.
3. (3) Indiana University School of Medicine-West Lafayette.
4. (4) Indiana University School of Medicine-Evansville.
5. (5) Indiana University School of Medicine-South Bend.
6. (6) Indiana University School of Medicine-Terre Haute.

7. (7) Indiana University School of Medicine-Muncie.
