"Family practice" means that medical specialty that:

1. (1) is called family practice; and

2. (2) provides personal physicians who:

   1. (A) serve as first medical contacts for patients;
   2. (B) provide a means of entering the health care system; and

   3. (C) accept responsibility for a patient's total health care.
