Ind. Code § 21-13-6-1

"Primary care physician"

As added by P.L.170-2009, SEC.4.

As used in this chapter, "primary care physician" means a physician who practices in any of the following areas:

(1) Family practice.
(2) Pediatrics.
(3) Obstetrics and gynecology.
(4) Internal medicine.
