After a school receives score reports, the school shall schedule a parent/teacher conference with the following:

1. (1) A parent of a student who requests a parent/teacher conference on the scores of the student.

2. (2) The parent of each student who does not receive a passing score on the test. The conference must include a discussion of:

   1. (A) the student's test scores, including subscores on academic standards; and

   2. (B) the proposed remediation plan for the student.
